A request is received when an individual or their agent submits a request through our online form, email, or our toll-free telephone number. A request may be denied if the individual submitted a request but did not complete steps to enable us to verify their identity, or their identity could not otherwise be verified. A request submitted by an authorized agent may be denied if the agent did not provide sufficient documentation to allow us to determine that the agent was authorized by the individual to submit the request.
